Wash apples and pat dry completely. Slice into ¼ to ½-inch thick rounds. Use a small round cutter or knife to remove the core from the center of each slice. Lay slices flat on a parchment-lined tray.
In a microwave-safe bowl, heat caramels with a splash of heavy cream (if using) in 30-second intervals, stirring between each, until smooth. Let cool slightly to thicken.
Using a spoon or offset spatula, spread a layer of melted caramel over each apple slice. Place the tray in the refrigerator for 5–10 minutes to set the caramel.
Melt chocolate chips in the microwave in 30-second bursts, stirring until smooth. Drizzle or spoon chocolate over the caramel-covered apple slices.
Immediately sprinkle chopped pecans on top. Add any extra toppings like sea salt or white chocolate. Refrigerate until set, about 5–10 minutes more.
